What is first-pass yield?

It is the number of units accepted without rework divided by the total entering the defined process.

Should reworked units count as good?

Not for first-pass yield. They can count in final yield after rework, but FPY exposes the original process loss.

Can FPY exceed final yield?

Normally no. Final yield may be higher after successful rework, while FPY remains the original first-pass result.

Why track scrap separately?

Rework and scrap have different causes and costs. Separating them makes corrective action and cost analysis more useful.
